Reminder for Summer Camp Dates - Medical Forms !!!
Medical Forms: From Cheryl Reeve: Hello everyone, Here is my weekly reminder that I need to have a completed health form and a copy of the front and back of your insurance card prior to the boys leaving for summer camp. For all the new scouts, you can get a copy of the form from our web site. There are sections that the parent needs to complete as well as your doctor. They will not allow the boys to attend summer camp unless this form is completed. Thank you to all the parents who have responded as to the status of their appointments/forms. If anyone has any questions or concerns, please do not hesitate to contact me.
